Senny Mayulu opened the scoring for Luis Enrique’s men before Guela Doue equalized before the break.
Strasbourg had missed a penalty earlier and thought they were on course to take all three points when Achraf Hakimi was sent off 15 minutes from time.
However, nine minutes from time, Nuno Mendes appeared to head home the winner for the 10-man Parisians.
This result places the European champions above Lens and in first place. For Strasbourg and O’Neil, they remain seventh.
How the action took place
Strasbourg had a glorious opportunity to take the lead 20 minutes after winning a penalty when the ball hit Marquinhos’ hand in the box.
Forward Joaquin Panichelli assumed shooting duties on goal, but his shot was stopped by Matvey Safonov.
It looked like Enrique’s decision to leave out some key members for the clash at the Stade de la Meinau could prove costly.
However, under the eyes of Ousmane Dembélé and Désiré Doué from the bench, PSG opened the scoring a few moments later.
After predictably having much of the ball in the early stages, the Ligue 1 champions broke the deadlock through 19-year-old Mayulu in the 22nd minute.
But the advantage lasted just five minutes when O’Neil’s men equalized after Guela Doue – Desire’s older brother – found the net from a cross from former Chelsea full-back Ben Chilwell.
After the break, Strasbourg looked bright before PSG finally regained control of the match despite being unable to break down their opponents.
With fifteen minutes remaining, Hakimi received his marching orders after VAR upgraded his booking for a challenge on Panichelli.
Despite their numerical disadvantage, the Parisians regained the advantage shortly after when Nuno Mendes headed in a cross from Warren Zaire-Emery.
The ten visitors finished the rest of the match to return to the top of the French elite.
